In September 2022, Emory University Hospital Midtown admitted Fernando Cluster for a critical procedure necessitated by an intracerebral hemorrhage. The surgery required the removal of a 28-square-inch section of his skull to relieve pressure on his brain. Doctors intended to store the removed bone and later reattach it.
However, when Cluster returned to the hospital two months after the procedure to have his skull piece replaced, hospital staff were unable to locate it. Hospital records indicate that the storage area for such bones had numerous unlabeled fragments, and doctors could not identify Cluster among them.
Poorly managed patient records hindered the search for Cluster's original skull piece. Notes from hospital staff indicated that several stored bone flaps had incomplete or missing patient identification. Hospital personnel explained, "We inspected the freezer where bone flaps are stored and could not find a bone flap with Mr. Cluster's patient identification."
